絲蟲病的發病與病變主要由成蟲引起,感 染期幼蟲亦起一定作用,與血中微絲蚴關系不大。病變發生發展 取決于絲蟲種類、機體免疫反應、感染頻度、感染期幼蟲進入人 體數量、成蟲寄生部位以及是否合并繼發感染等因素。在幼蟲進 入機體發育為成蟲過程中,幼蟲與成蟲的代謝產物以幼蟲蛻皮液 、蟲體子宮內排泄物以及死蟲的裂解物均可引起局部淋巴系統的 組織反應及全身過敏反應,表現為周期性發作的淋巴管炎、淋巴 結衍及絲蟲熱等。晚期表現則為淋巴組織病理改變及繼發細菌感 染的結果。目前認為,免疫機制是產生病理改變的主要原因。免 疫反應與淋巴系統的損害有關,急性淋巴管(結)炎被認為屬1型 或3型反應,組塞性絲蟲病組屬4型邊帶反應。早期以滲出性炎癥 為主,淋巴結充血,淋巴管壁水腫,管腔內充滿粉紅色蛋白質液 體和嗜酸性粒細胞。繼之,淋巴結和淋巴管內出現肉牙腫性反應 ,肉牙腫中心為變形的成蟲和嗜酸性粒細胞,周圍繞以纖維組織 和上皮樣細胞,上有膽量淋巴細胞和漿細胞,類似結核結節。
